Key Patient-Monitored Tests by Condition (Prioritised for Digital Access)
This appendix lists the conditions and clinical results most important for patients in monitoring their health and
managing long-term conditions. The items in sections 1–6 are widely regarded as safety-critical and should be
prioritised for clear, consistent, and timely visibility in digital patient record systems.
It is recognised that clinical teams remain the ultimate arbiters of which results should be shared, and that some
local variation may exist in what is considered most appropriate for patient access. The list below is therefore
proposed as a basis for discussion and refinement by clinicians.
1. Diabetes (Type 1 & 2)
1.1 HbA1c – long-term glucose control.
1.2 Fasting / random plasma glucose – diagnosis & short-term monitoring.
1.3 Urine albumin:creatinine ratio (ACR) – early kidney damage detection.
2. Chronic Kidney Disease (CKD Stages 1–5)
2.1 eGFR – kidney function / CKD staging.
2.2 Serum creatinine & urea – renal function trends.
2.3 Electrolytes (Na, K) – safety monitoring.
2.4 Urine ACR – proteinuria monitoring.
3. Cardiovascular Risk & Hypertension
3.1 Cholesterol profile – Total, HDL, LDL, triglycerides.
3.2 Blood pressure readings – clinic & home BP.
3.3 QRISK / CVD risk scores – derived from results & demographics.
4. Liver Disease / Medication Safety
4.1 LFTs: ALT, AST, ALP, bilirubin, albumin.
4.2 INR / clotting screen – for liver disease or anticoagulation monitoring.
5. Thyroid Disease
5.1 TSH – primary screening/monitoring.
5.2 Free T4 / Free T3 – for treatment adjustment, hyperthyroidism follow-up.
6. Haematology / Anaemia
6.1 Full blood count (FBC): Hb, WBC, platelets, MCV.
6.2 Ferritin – iron stores.
6.3 Vitamin B12 / Folate – macrocytic anaemia work-up.
7. Rheumatology / Inflammatory Disease
7.1 CRP – inflammatory marker.
7.2 ESR – chronic inflammatory conditions.
7.3 Uric acid – gout monitoring.
8. Heart Failure / Cardiac Disease
8.1 BNP / NT-proBNP – diagnosis and monitoring.
8.2 Echo / ECG reports (summaries).
9. Cancer Surveillance
9.1 PSA – prostate monitoring.
9.2 CA125 / CEA / other markers – ovarian, bowel, and other cancers.
9.3 Cytology / histology report summaries.
10. Drug & Therapy Monitoring
10.1 Methotrexate / DMARD bloods – FBC, LFT, U&E.
10.2 Lithium levels – therapeutic monitoring.
10.3 Clozapine monitoring – WBC counts & metabolic monitoring.
10.4 Other high-risk drug monitoring (e.g. amiodarone, valproate).
11. Nutrition / Metabolic Monitoring
11.1 Vitamin D – bone health.
11.2 Calcium, phosphate, magnesium – metabolic bone disease, CKD.
12. Infectious Disease Monitoring
12.1 HIV viral load / CD4 counts.
12.2 Hepatitis B & C serology / viral load.
13. Respiratory Disease (Asthma / COPD)
13.1 Spirometry reports – FEV1, FVC trends.
13.2 Peak flow trends – for patient self-management.
13.3 Oxygen saturation (if monitored).
14. Weight & BMI
14.1 Weight / BMI trends – metabolic syndrome, diabetes prevention programmes.
